Disclaimer: This is not online booking.  Our staff will first review your information and contact you to set up an appointment. If a patient does not comply with filing out all the paperwork, we will close their chart.  Please do not use this feature just to create an empty chart. All forms must be completed for insurance to be verified and appointment scheduled.  Within a week, charts that not completed with be deleted.

New policy: We are unable to schedule new or follow up appointments, without an active credit card on file to payment for services rendered. This is due to a result of patients not paying for services that were rendered.

 

Our staff does not feel that it is appropriate to continually contact patients after evaluations and follow up have been completed. We will cancel all appointment without this insurance on file. It is formally signed by each patient, which is required for various payments including co-pay, co-insurance, deductible, or self-pay payments.

No Show/Last Minute Cancellation

 

Again, as our policy has stated:

​

1) No Show/Last Minute Cancellation: patient would be responsible for the full charged amount as stated in the financial agreement.

​

2) Cancellation Policy - New must patient must cancel with 48 hours of the schedule appointment; Follow up patient much cancel 24 hours of the schedule appointment.

After business hours, If you are in a crisis or concerned about your safety, in a life threatening situation, please call 988 or 911. If you are have adverse side effects, please proceed to the nearest ER or Urgent Care. All non-urgent issues should be messaged to Dr. Reddy and we will contact you in the next business day.
